Nurse Practitioner/Physician Assistant

Job Summary
We are seeking a dedicated and energetic Nurse Practitioner or Physician Assistant to join our dynamic healthcare team. In this role, you will provide comprehensive patient care in our newly opened Fairmont, WV outpatient clinic. Your expertise will support diagnosis, treatment, and management of diverse medical conditions, ensuring high-quality, patient-centered care. T

Responsibilities

• Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans.
• Manage acute and chronic conditions by prescribing medications.
• Utilize electronic health record (EHR) systems such as One Touch to document all patient interactions accurately and efficiently.
• Collaborate with specialists in behavioral health, and other fields to coordinate comprehensive care.
• Provide triage assessments via telehealth or in-person visits to prioritize urgent cases and ensure timely interventions.
• Support specialized services such as hospice & palliative medicine, memory care for dementia patients, or post-acute care programs.
• Maintain compliance with HIPAA regulations and ensure accurate medical documentation in accordance with NCQA standards.

Experience

• Proven experience working in diverse healthcare environments including hospital medicine, emergency medicine, ICU or trauma centers (Level I or II trauma experience preferred).

Pay: $95,000.00 - $115,000.00 per year
