Position Overview
AvedaMed, LLC is seeking a Nurse Practitioner or Physician Assistant to join our
growing medical practice. This role will support a blended care model involving
outpatient clinic coverage, hospital rounding, and nursing home/SNF coverage.
The ideal candidate is clinically strong, dependable, professional, and comfortable
working in a fast-paced environment with physicians, clinical staff, patients, families,
hospitals, and facility partners.

- Affiliated nursing homes/SNFs
Schedule
- Full-time position.
- Schedule may include a combination of clinic, hospital, and nursing home coverage.
- One Saturday per month is required.
Compensation
- $120,000–$140,000 annually, based on experience.
- Productivity and/or performance bonus opportunity may be available.
Responsibilities
- Provide patient care in the outpatient clinic setting.
- Assist with hospital coverage and rounding.
- Provide nursing home/SNF patient coverage as assigned.
- Perform histories and physicals, follow-up visits, and clinical assessments.
- Document encounters accurately and in a timely manner in the EHR.
- Communicate with physicians regarding patient care plans.
- Coordinate with clinical staff, facility staff, patients, and families.
- Support continuity of care between clinic, hospital, and facility settings.
- Assist with quality measures, care gaps, and clinical follow-up.
- Maintain professionalism, reliability, and compliance with practice policies.
Qualifications
- Active New Jersey Nurse Practitioner or Physician Assistant license.
- Board certification as applicable.
- Ideally, Preferred experience in internal medicine, family medicine, geriatrics,
- hospital medicine, or post-acute care. However, willing to train the right
- candidate.
- Strong clinical judgment and communication skills.
- Ability to work independently while collaborating with supervising physicians.
- Comfortable with outpatient and facility-based care.
- EHR experience preferred.
Preferred Candidate
- Experience in internal medicine, family medicine, geriatrics, hospital medicine, or post-acute care preferred.
- Dependable and organized.
- Comfortable managing multiple care settings.
- Strong documentation habits.
- Professional with patients, staff, and outside facilities.
- Interested in helping build and improve a growing practice.
Pay: $120,000.00 - $140,000.00 per year
