Key Responsibilities

• Complete comprehensive wound assessments and develop individualized care plans
• Prescribe medications and treatments as clinically appropriate
• Provide in-person and telehealth visits (as permitted by state regulations)
• Document patient care thoroughly and accurately
• Deliver care in patient homes, with occasional visits in SNFs or rehab facilities

Qualifications

• Active Nurse Practitioner (FNP, APRN) or Physician Assistant license in Texas
• Master’s degree from an accredited NP/PA program
• Preferred: 1+ year experience in primary care, internal medicine, urgent care, or wound care
• Preferred: ER or ICU experience
• Strong clinical judgment, communication, and organizational skills
• Compassionate, self-motivated, and patient-focused

Schedule

• Monday–Friday, daytime hours only
• No evenings, weekends, or on-call shifts
