Job Summary
We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team in this fully remote position. In this pivotal role, you will provide high-quality, patient-centered care for multiple subacute rehab and skilled nursing facilities. Your expertise will support comprehensive assessments, diagnosis, treatment planning, and ongoing management of patients.
This position is for the 7PM - 7AM (3 nights x 12 hours).

Responsibilities:
During hours of work, there will be times answering the call line and when not answering the call line there will be other items to complete.
- Cover our on-call provider line, which currently covers multiple subacute rehab and skilled nursing facilities in across multiple states.
- Review labs, radiology reports, medications, consults, pharmacy recommendations.
- Prescribe medications including narcotics and controlled substances when needed.
- Prescribe and order a variety of IV medications, IV fluids.
- Order diagnostic testing when needed (labs, x-rays, fluids, UA etc.).
- Send to ED or refer to specialists when needed.
- Other remote work as delegated by Director of Remote Provider Services
Requirements
- Valid NY state licensure as a Nurse Practitioner or Physician Assistant with current certification (e.g., NCCPA certification for PAs).
- Current/Valid NY DEA license
- Proven experience in subacute rehab and skilled nursing facilities
- Strong familiarity with EMR/EHR systems
Nice to haves:
- Licensed already in Massachusetts and/or, Illinois and/or Pennsylvania
