Job Title: Nurse Primary Assessor

Overview:
This is the Nurse Primary Assessor position, in which you will be going to potential new clients, meet the responsible parties/ family members, assess the client and the work load being requested, discuss the care plan expectations from the family and adjust tasks based on your professional analysis. You will also be responsible to fill out an Individual Care Plan, once leads commit to becoming clients. Ideally, you will be a part of a team of 2 nurses, who can collaborate at their own discretion the division of labor. This role will be paid a flat rate per client meeting at $50, with a potential mileage bonus if the commute is longer than 40 minutes. Individual Care Plans will also be paid out at $50 per plan. Please be aware, you will be asked to drive to various locations in and around Austin, including but not limited to Round Rock, Georgetown, Hutto, Pflugerville, Bastrop, Manor, Buda, etc. Anywhere a client lead lives!

Responsibilities as an assessor:
- Conduct thorough assessments of patients' medical conditions and needs
- Representing the company as the first in person contact for the family.

-Ensuring enough data gathered to sufficiently create an ICP.

- Assess patient's condition

- Assess, discuss, and adjust the care plan according to the needs of the client and the family's requests for care.

-Report findings and analysis to office staff.

Requirements:
- Valid nursing license
- Experience in elderly patient care
- Strong understanding of medical terminology and procedures
- Excellent communication and interpersonal skills
- 4+ years of experience in caregiving or with senior care. Experience with home health care and long term care of seniors is ideal, but not required.
- Ability to drive to various locations within a certain range of area ( Bastrop - Georgetown - Pflugerville - North Austin area.)
