Nurse Scheduling Coordinator/ Clinical Support

Job Summary:

The Scheduling Coordinator:

· Coordinates field nursing visits and therapy scheduling process for all service areas.

· Routinely interacts with clients, family members, contractors, community providers, and administrative and clinical staff.

Essential Functions:

· The SCHEDULING COORDINATOR complies with all organizational policies and procedures regarding ethical business practices.

· The SCHEDULING COORDINATOR acts as point of contact for interactions with clients, family members, staff, contractors, and community providers to ensure uninterrupted care of client.

· The SCHEDULING COORDINATOR assigns evaluation visits to appropriate clinical personnel and maintains open line of communication with field staff.

· The SCHEDULING COORDINATOR processes verbal requests and frequencies as received from admitting nurse and/or RNCM.

· The SCHEDULING COORDINATOR processes missed, reschedules and declined visits as instructed by DON and/or RNCM.

· The SCHEDULING COORDINATOR runs the pending authorization report and escalates problems to RNCM or DON for review.

· The SCHEDULING COORDINATOR communicates with contract therapy companies to ensure timely patient visits as appropriate.

· The SCHEDULING COORDINATOR reviews and reacts in a timely manner to daily and weekly reports as assigned.

· The SCHEDULING COORDINATOR ensures patient continuity of care.

· The SCHEDULING COORDINATOR works closely with the Intake and Authorizations Departments in regards to admissions, recertification, discharges, and insurance authorizations.

· The SCHEDULING COORDINATOR works in conjunction with , DON, ADON and RNCM to assist in resolution of client needs in regards to admissions and insurance authorizations.

· The SCHEDULING COORDINATOR may assist in completing and transmitting orders for contractors.

· The SCHEDULING COORDINATOR may assist in informing the physician and other personnel of changes in the patient’s condition and needs.

· The SCHEDULING COORDINATOR attends all mandatory meetings.

· The SCHEDULING COORDINATOR participates in training and in-service programs to enhance their skills and knowledge.

· The SCHEDULING COORDINATOR may serve on committees and participate in the development of systems to improve care coordination.

· The SCHEDULING COORDINATOR conducts themselves in a professional manner, both verbally and nonverbal.

· The SCHEDULING COORDINATOR promotes a positive attitude at all times.

· The SCHEDULING COORDINATOR performs other duties as assigned or as needed.

Education:

· High school diploma or GED

. LVN License, graduate from an accredited nursing program

Training Requirements:

. Licensed LVN preferred

· A minimum of one year related experience preferred.

· Knowledge of Medicare, Medicaid, and private insurance preferred.

· Proficiency in word processing and computer skills.

· Training or experience in human relations, customer service, time management, and organizational skills.

Knowledge Requirements:

· Must be able to read, write, speak and understand the English language.

· Understanding of HHA basic functions and goals.

· A good understanding and knowledge of Medicare and Medicaid policy, and private insurance.

· Able to make independent decisions when circumstances warrant such action.

Professional Requirements:

· Maintains and ensures HIPAA compliance.

· Responsible and accountable to the Administrator and DON regarding time availability and performance.

· Maintains good rapport and cooperative working relationships with patients, physicians, referrals sources, regulatory agencies and staff.

· Adheres to company dress code.

· Attends annual review and departmental in-service training.

· The applicant does not have record of a crime, offense or misdemeanor as defined in 97.241 of the Texas Administrative Code, Title 40, and Chapter 97.

· The candidate must adhere to high personal and professional ethical values.

· The candidate agrees to adhere by THCS Mission and professional behavior expectations.

Job Type: Full-time

Pay: $30.00 - $32.00 per hour
