Responsibilities:

- Monitoring and Reporting
- Respiratory Status
- Secretions: Monitor the amount, color, and consistency of secretions, reporting any abnormal changes
- Monitoring and Reporting
- Respiratory Status: Observe the patient's breathing patterns, rate, and effort, and monitor oxygen saturation levels.
- Secretions: Monitor the amount, color, and consistency of secretions, reporting any abnormal changes (e.g., thick, foul-smelling, or blood-tinged secretions) to the nurse immediately.
- Infection Signs: Watch for and immediately report signs of infection at the stoma site, such as redness, swelling, pain, or purulent drainage.
- Complications: Promptly notify medical staff of any signs of distress, difficulty breathing, or if the tracheostomy tube becomes dislodged.
- Patient Support
- Communication: Assist the patient with communication by providing aids like writing boards, picture boards, or using gestures, as speaking is often difficult with a new tracheostomy.
- Emotional Support: Provide reassurance and emotional support to help the patient and family adjust to the physical and emotional challenges of living with a tracheostomy.
- Positioning and Comfort: Position the patient in a semi-Fowler's position to promote optimal breathing and comfort.
- Oral Care: Provide regular oral hygiene, as natural airflow is disrupted and can lead to dry mouth and infection risk.
