Job Summary

Responsibilities
- Deliver comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using EMR (Electronic Medical Record) systems such as Epic or Cerner.
- Administer medications, injections, and other treatments following established protocols and HIPAA regulations to ensure patient safety.
- Manage complex care needs such as ICU procedures, trauma response in Level I Trauma Centers, post-acute care, hospice support, and dementia or Alzheimer's care.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to each patient’s condition and needs.
- Perform specialized procedures including catheterization, tube feeding management, sterile processing, wound suturing, and spinal taps when necessary.
- Conduct patient education on health coaching topics like pain management, medication administration, nutrition, discharge planning, and chronic disease management.
- Maintain accurate medical documentation of all patient interactions, assessments, treatments, and progress notes in compliance with medical terminology standards and ICD coding (ICD-9/ICD-10).
Skills
- Proven experience working with diverse patient populations in settings such as hospitals (including ICU and emergency room), nursing homes, outpatient clinics, or home & community care environments.
- Strong knowledge of physiology, anatomy, infectious disease control protocols, and emergency procedures including airway management and ventilator support.
- Proficiency with EMR/EHR systems like Epic, Cerner or Athenahealth for efficient documentation and case management.
- Ability to assess patients’ conditions accurately through vital signs monitoring and diagnostic evaluations; capable of triage in urgent or emergency situations.
- Excellent communication skills for working with patients of all ages—including pediatrics and neonatal ventilator care—and collaborating effectively with healthcare teams.
- Knowledge of HIPAA compliance standards for protecting patient confidentiality; experienced in medical records management and discharge planning processes.
