Albany Family Medicine (AFM), part of Community Care Physicians (CCP) — the Capital Region's first and largest independent multispecialty medical group — is seeking a Nurse Practitioner or Physician Assistant to join our dedicated primary care team.

Position Overview

As an Advanced Practitioner at Albany Family Medicine, you will provide comprehensive family medicine care in a supportive, team-based environment that includes attending physicians, residents, nurses, and clinical support staff. You will practice to the full extent of your license, supported by CCP's extensive specialty network and resources.

This position offers no after-hours call, allowing you to maintain a healthy work–life balance while focusing on delivering high-quality, patient-centered care.

Responsibilities

• Provide primary care services for patients of all ages, including acute, chronic, and preventive care
• Perform patient assessments, diagnose conditions, and develop individualized treatment plans
• Order, interpret, and follow up on laboratory tests and diagnostic studies
• Prescribe medications and manage ongoing treatment plans
• Educate patients and families on wellness, prevention, and disease management
• Collaborate closely with physicians, residents, and interdisciplinary team members
• Support a teaching environment by working alongside and contributing to the education of family medicine residents

Requirements:

• Current New York State NP or PA license.
