JFSA has an immediate opening for a full-time care coordinator within our Older Adult Department. Understanding, experience, and sensitivity working with Holocaust survivors is preferred.
Duties and Responsibilities:

Provides compassionate care coordination to a caseload of clients and their families.

Ensures that the client receives the appropriate and needed services; monitors service provision to ensure the client is satisfied with the quality and frequency of the services. Helps to mediate any issues or concerns between the client and the service provider(s).

Assists and encourages participation in the development of an individualized, comprehensive plan for each client taking into consideration the person’s history, cultural influences, and values.

Maintains a clear and ongoing understanding of the client’s finances to assist client in current and future financial needs planning.

Acts as a liaison between the client, family members, and all agency providers of service in order to coordinate care and ensure client satisfaction with care.

Coordinates care for significant events such as inpatient hospitalizations or sudden health changes.

Maintains communication with clients, family members, and other services providers to assure responsiveness within 24 hours. Responds to client and family needs in sensitive fashion.

Completes documentation in a timely fashion for all clients as required. Ensures all documentation is received and entered correctly into the electronic health record.

Completes billing documentation in a timely fashion and in accordance with service standards.

Meets productivity targets.

Minimum Qualifications:
-
Bachelor’s Degree in Social Work or a related field.
-
Master’s Degree in Social Work preferred.
-
Minimum 2 years work experience with seniors.
-
1-3 years case management or care coordination preferred.

Maintain both a valid Ohio Driver’s License, and a driving record that meets JFSA’s insurance company standards (e.g. less than 2 incidents over a 3-year period).

Maintain professional licensure as required by the State of Ohio licensing board.
