Schedule: Monday to Friday | Light Call | 40 hrs/week

• In-person visits for chronic disease care
• Collaborative care, partnering with PCPs, cardiologists, and facility teams to improve patient’s outcomes.

What You’ll Do – Your Role

• Conduct in-person, preventive cardiology and chronic disease-focused visits in Assisted Living and Skilled Nursing settings
• Monitor patients’ chronic cardiac conditions daily, using remote technology
• Collaborate with primary care providers, other cardiologists, and our internal team.
• Document clinical notes and treatment plans with attention to detail
• Educate patients, families and staff on heart health, prevention, and wellness
• Contribute to a culture of excellence and patient-centered care

What You Bring – The Ideal Candidate, Active Nurse Practitioner License (Florida). Strong background or interest in Cardiology or Geriatrics Experience, working in long-term care settings preferred. Proactive care. Integrity, autonomy, and respect. Excellent communication and collaboration skills. Tech-savvy and comfortable using remote monitoring tools. Organized, detail-oriented, and passionate about prevention. Willingness to travel locally between facilities
