Occupational Therapist

To ensure the quality and safe delivery of occupational therapy services. To assist in the home care services in accordance with company philosophy and established standards of home care and occupational therapy practice.


Essential Functions:

In accordance with established Agency standards, parameters of responsibility, clinical and administrative policies and procedures:

  • Participates in the development and maintenance of treatment plans.
    • Assists the physician in evaluating home care patients’ level of function by applying diagnostics and prognostics procedures.
    • Participates with other clinical staff in assessing patient needs and developing an appropriate plan of care.
    • Completes occupational therapy assessment note in a timely manner.
    • Participates in case conferences as assigned.
    • Involves patients and family members in developing and implementing the plan of care whenever possible.
    • Communicates recommendations and requirements for continuation and/or additional occupational therapy services to appropriate individuals and obtains a physician’s order for any needed special equipment.
    • Instructs other clinical staff members in certain phrases of the therapy program in which they may assist the home care patient.
  • Implements current occupational therapy practice.
    • Guides home care patients in the use of therapeutic, creative, and self-care activities for the purpose of improving function.
    • Helps home care patients adjust to physical limitations and maximize capabilities.
    • Submits timely, complete clinical records documenting services rendered and observations about patient’s progress.
    • Participates in discharge planning activities and prepares a written discharge summary.
    • Implements appropriate infection control and safety measures and reports all hazards and accidents in a timely and accurate manner.
    • Maintains confidentially of patient information.
    • Adheres to all Agency policies regarding schedule changes and/or missed visits.
  • Uses equipment and supplies safely and efficiently.
    • Demonstrates accurate and efficient utilization of equipment and supplies.
    • Communicated information regarding malfunctioning equipment and/or inadequate supplies to appropriate
    • individuals.
    • Communicated information regarding safe and efficient use of equipment and supplies to patients and families.
  • Participates in professional growth and development.
    • Attends Agency in-services.
    • Develops and presents educational programs for Agency clinical staff as requested.
    • Demonstrates current knowledge of the principles of occupational therapy practice.
    • Maintains current professional licensure.
    • Participates in the Quality Assessment and Improvement program as requested.
    • Serves as a member of the Advisory Board and the Clinical Record Review Committee as requested.

Physical Requirements: Ability to perform tasks involving physical activity, which may include heavy lifting and extensive standing and bending. Visual/hearing ability sufficient to comprehend written/oral communication. Ability to deal effectively with stress.


Working Conditions: Works in office areas as well as throughout Independent Living Facilities and residential homes. Sits, stands, bends, lift, and moves intermittently during working hours. Interacts with residents, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances. Is subject to hostile and emotionally upset residents, family members, staff, visitors, etc. under all conditions/circumstances. Is involved in community/civic health matters/projects. May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B viruses. Communicates with the medical staff, nursing staff, and other unit supervisors. Maintains a liaison with residents, their families, support staff, etc. to assure that the residents' needs are continually met.

Qualifications: One year recent experience as an Occupational Therapist. Must have a car available.


Certification and License Requirements: Currently licensed to practice in the State of Virginia. Registered by the American Occupational Therapy Association.
