PRN Occupational Therapist (OT)

Job Description:

The Occupational Therapist plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Responsibilities:

  • Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.

  • Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents' functional mobility.


  • Treatment: Administer therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.


  • Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.

  • Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( InnovaHealth/Net Health/PCC).

  • Interdisciplinary Collaboration: Collaborate with the rehab team, including Physical Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.

Qualifications:

  • State licensure as an Occupational Therapist.

  • EMR and MS office applications.

  • CPR/BLS certification.

  • Knowledge of Section GG data collection and documentation requirements.

  • Strong clinical and interpersonal skills.

  • Excellent written and verbal communication skills.

  • Ability to supervise assistants, technicians/aids and student/interns.

  • Modality Certification +

  • Experience in LTPAC/LTC clinical programs are always a plus
