Job Overview
Ohman Family Living is hiring a Home Health Occupational Therapist OT to provide in home therapy services across Geauga County. This role focuses on helping adult and geriatric patients improve function, safety, and independence at home.

Key Responsibilities

• Perform occupational therapy evaluations in patient homes
• Develop and implement individualized treatment plans
• Provide skilled OT services to improve ADLs and daily function
• Complete home safety assessments and recommend adaptive equipment
• Educate patients and caregivers on therapy plans and routines
• Collaborate with RN, PT, and interdisciplinary care team
• Maintain accurate and timely EMR documentation
• Follow Medicare and home health regulations

Qualifications

• Licensed Occupational Therapist OT in Ohio
• Graduate of an accredited Occupational Therapy program
• Home health or rehab experience preferred
• Experience with adult and geriatric patients preferred
• Valid driver’s license and reliable transportation
• Strong communication and time management skills
• Comfortable working independently in patient homes
