Duties
- Respond promptly to on-call requests, providing expert nursing assessments and interventions for hospice patients in their homes or community settings.
- Conduct comprehensive patient evaluations, including vital signs, symptom management, and physiological assessments tailored to individual needs.
- Collaborate with physicians, social workers, and other healthcare professionals to develop or adjust personalized care plans that prioritize patient comfort and dignity.
- Manage medication administration, including pain control and symptom relief medications, adhering strictly to safety protocols and HIPAA regulations.
- Educate patients and families on disease progression, medication use, and comfort measures to empower them during end-of-life care.
- Document all patient interactions thoroughly in medical records, ensuring accurate case management and continuity of care.
- Coordinate discharge planning and post-acute care services as needed, facilitating smooth transitions across care settings.
Qualifications
- Valid nursing license with current registration in the relevant state or jurisdiction.
- Extensive clinical experience in critical care environments such as ICU, Emergency Room, Level I or Level II Trauma Centers, or Medical-Surgical units.
- Proven expertise in hospital medicine, post-acute care, outpatient clinics, or home health nursing.
- Strong knowledge of physiology, infectious disease management, behavioral health issues, geriatrics, pediatrics (PICU), and trauma care principles.
- Experience with medical records systems, diagnostic evaluation procedures, intake processes, discharge planning, utilization management, and case management.
- Familiarity with HIPAA regulations and medical terminology essential for accurate documentation and patient confidentiality.
- Ability to perform thorough patient assessments including vital signs and symptom evaluation in acute or outpatient settings.
