Current Beacon Pharmacist under the title of "Clinical Pharmacy Specialist" is leaving and willing to stay PD to assist; at least until we are able to recruit his replacement.Obtains and/or maintains Epic Willow and Beacon certifications.
Leads and develops pharmacy clinical informatics services.
Completes drug utilization reviews.
Participates in the design, collection and reporting of adverse drug reactions, drug utilization reviews, medication errors and pharmacy quality initiatives. Implements strategies to minimize medication errors.
Leads the training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians.
Provides and demonstrates expertise in a specialized area of pharmacy practice (operational and/or clinical).
Effectively and proactively intervenes in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.
Maintains appropriate controlled substance records. Works with the Pharmacy Director appropriate manager to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards. Participates in controlled substance reviews and audits.
Complies with all regulatory standards. Works with Pharmacy Management to identify and develop processes to meet or exceed all regulatory standards. Participates in quality and regulatory reviews and audits.
Prioritizes work and leads others to comply with medication turn-around time policy and procedure. Identifies areas for improved efficiency. Assists in implementation of clinical and operational process changes.
Ensure the ordering, verification and dispensing process for chemotherapy and supportive care medications follow institutional policies, NCCN guidelines, and regulatory standards.
Provide clinical consultation to oncology providers and staff on treatment plans, dosing, and supportive care.
Participate in oncology tumor board meetings, case reviews, and Pharmacy & Therapeutics Committee meetings.
Provides advanced technical support for automated dispensing machines and other computer systems, including the smart pump library and CADD pump records.
Build, maintain, and optimize Beacon treatment/therapy plans, order sets and medication records in Epic, including outpatient, inpatient and research protocols.
Perform testing, validation, and troubleshooting of Beacon workflows, upgrades, and enhancements.
Collaborate with informatics team by actively monitoring assigned tickets, documenting activities thoroughly, and resolving issues within the established service levels in a consistent manner.
Ensure compliance with USP <797>, <800>, Joint Commission, ASHP, and institutional standards.
Participates in prioritization, creation, and/or validation of new oncology drug entries in EPIC and relevant pharmacy automation systems.
Assist with development and maintenance of oncology-related policies, procedures, and workflows.
Participate in medication safety initiatives, quality improvement projects, and audits related to oncology services through data analytics.
Provide education and training to pharmacists, providers, nurses, and staff on Beacon workflows and oncology protocols.
Serve as a resource for Epic Beacon-related questions and optimization opportunities.
Acts as manager in charge as assigned.
Provides review and guidance on physician order sets and other evidenced based medication use guidelines.
Participates in all aspects of managing the drug formulary.
Recommends and participates in the evaluation of new technologies for safety and integration with existing systems.
Responsible for system activities related to upgrades and optimization of pharmacy and beacon applications including review of changes, build, testing, and training that may be required. Accounts for application dependencies and other workflow. concerns. Establishes and executes a communication plan related to upgrade/optimization activities to impacted areas.
Knowledgeable of downtime processes. Actively participates in testing of downtime processes. Demonstrates broad understanding of downtime as it affects the enterprise. Ensures systems and workflows are adequate to recover from planned or unplanned downtime events.
Registered Pharmacist in the State of Florida and Board Certification preferred for a Pharm D. candidate and required for a B.S Pharmacist Candidate. Board Certification in specialty area of practice preferred. Beacon Certification preferred.
Pharm.D. from an ACPE accredited School of Pharmacy or B.S. Pharmacy Degree from an ACPE accredited School of Pharmacy WITH a required Board Certification.
Specialized Residency (PGY-2), or Fellowship in oncology, or equivalent practice experience (completion of 4 years of practice experience in oncology or completion of a PGY-1 Residency plus 2 additional years of practice experience in oncology) Track record of exceptional performance in informatics, leadership, clinical practice, and education in specialty area of practice. Fully meets ASHP Residency Preceptor Requirements.
