A premier orthopedic group is seeking a motivated and skilled Outpatient Physical Therapist (PT) to deliver outpatient-level care directly in patients’ homes. This unique position offers the opportunity to collaborate closely with orthopedic surgeons and the hospital’s management company, ensuring a streamlined, high-quality care experience from surgery through recovery.

Physical Therapist (PT) Duties:

  • Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes

  • Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care

  • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.

  • Educate patients and their families about treatment options, exercises, and preventive measures.

  • Conduct comprehensive evaluations and implement tailored treatment plans

  • Document all care in alignment with outpatient standards and regulatory requirements

  • Participate in ongoing case reviews and care team meetings

  • Work with and supervise PTAs

Physical Therapist (PT) Experience & Qualifications:

  • Current and valid Physical Therapist license in TX

  • Minimum 1 year of outpatient orthopedic or home health experience preferred but not required

  • If new grad, will provide training and mentorship

  • Strong interpersonal and communication skills

  • Self-directed, with the ability to work independently in the field

  • Reliable transportation and willingness to travel within service area

  • Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
