Job Description
The Outpatient Coordinator is responsible for ensuring contract requirements are met as defined by the Alcohol and Drug Program administration and other agencies. The Outpatient Coordinator will assist in collaboration with the Program Director, the coordination of outpatient services to individuals with substance use and co-occurring disorders. This includes a collaboration with the Director of Business Development to outreach, development of referral networks and provision of direct services to ensure census and budgetary metrics are met. The Outpatient Coordinator works with a multi-disciplinary treatment team to develop and implement the treatment plan and/or treatment strategy to ensure clinical integrity.
Responsibilities
In addition to all components of the above position summary the following “Essential Duties and Responsibilities” are essential to successful performance in this position.
· Monitors and ensures contract requirements are met as defined by the Alcohol and Drug Program Administration and other agencies as appropriate.
· Ensures all required reports for governing or funding agencies are completed in an accurate and timely fashion.
· Responsible for establishing and maintaining relationships with multiple referral sources.
· Attend and participate in designated meetings, conferences, speaking engagements and media appearances as required.
· Collaborate with the admissions team to facilitate admissions process for assessments and referrals.
· Accompany other agency staff to networking events and opportunities including, but not limited to, agency site visits, community events, and local provider offices.
· Work with account specialists to maintain database to confirm copayments.
· Collaborate with Compliance Officer to ensure documentation compliance, including sign-in sheets and treatment plans, ASAM and Authorizations are submitted timely by all staff.
· Provide direct clinical work/counseling services to patients in group therapy settings and 1:1 individual session.
· Complete and submit all clinical documentation (group notes, individual session notes, treatment plans, treatment plan updates, utilization reviews, and assessments) in a timely manner as outlined in Casa’s agency policies and funding source contracts in patients’ Electronic Health Record in AURA.
· Maintain schedule of activities in EMR; monitor alerts in EMR; failed activities, missing treatment plans, activities not statuses, etc. and resolves issues.
· Collaborate with Compliance Officer to audit department client charts to maintain clinical compliance.
· Work with inter-disciplinary team to develop and implement treatment plans/strategies.
· Consult with and reports to direct supervisor or other agency leaders to resolve crises and minimize risk to the agency.
· Facilitate managing day-to-day operations of the Outpatient Program, including ensure groups are running on-time, sign-in sheets are collected, and program material/content is prepared in advance.
· Completes and submits progress reports, sessions notes, utilization reviews and authorization extension request necessary for patients’ funding sources in a timely manner.
· Assist patient in creation of discharge and aftercare plan that includes relapse prevention plan, referrals for external support, and aftercare appointments.
· Establishes and maintains professional and appropriate relationships with patients, patients’ support systems, and community including referral sources.
· Effectively communicate pertinent information regarding current patients to rest of treatment team including Clinical Supervisor, Outpatient Therapists/Counselors, Clinical Director, Program Director Program Recovery Specialist, and Medical Director.
· Attend and actively participate in weekly Clinical Supervision meetings that include presenting patient case conceptualizations for constructive feedback and demonstration of maintenance of current caseload, documentation, and patient progress.
· Assist in taking inventory of department supplies and items that need to be ordered; assists with facilitating department orders.
· Assist in keeping record of client outcome measures (e.g., PHQ-9s, Quality of Life surveys) and client satisfaction surveys; collaborate with other agency staff to create and submit quarterly data related to client outcome measures.

Education, Experience, and Credentials Required

· Proficiency in MS Office systems.

· Bilingual in Spanish preferred.
Qualifications:
· Master’s degree in social work, Marriage and Family Therapy or related field preferred
· SUD Certification or within 6 months of job offer.
· Must maintain licensure as a condition of employment.
· Experience with SAPC, insurances, and other governing bodies preferred.
· Evidenced-Based Trained/Certified in TF-CBT, SS Preferred
· Must maintain a valid California Drivers' License, have proof of automobile insurance and maintain a safe driving record based on Casa Treatment Center policy.
