About Vijapura Psychiatry
Vijapura Psychiatry is a Harvard-trained physician-led, medically integrated outpatient psychiatry practice serving Northeast Florida since 1991. Our model combines expert psychopharmacology, integrated psychotherapy, medical collaboration, and lifestyle-based interventions.

We are launching a structured 12-month Outpatient Psychiatry Fellowship for Psychiatric Nurse Practitioners (PMHNPs) and Physician Assistants (PAs) who are committed to mastering outpatient psychiatric medicine under direct physician mentorship.

This is a deliberate, mentored transition into excellent outpatient practice.

Position Overview
This 12-month fellowship is designed to bridge the gap between graduate training and independent outpatient psychiatric practice through immersive supervision, structured didactics, and progressive clinical responsibility.

Fellows train directly under:

  • Dr. Sagar Vijapura, MD – Medical Director
  • Lead APP (trained in-house, 5+ years with practice)
  • Dr. Priyanka Vijapura, MD – Internal Medicine (former Mayo Clinic faculty)
  • Additional physician faculty for case conferences and clinical trial review

Clinical Setting

Hybrid model including:

  • In-office visits at our primary location on Wednesdays

  • Telemedicine visits

Clinical days: Monday–Thursday, 8am to 5pm
Friday mornings: Structured didactics and case conferences
Friday afternoons: Independent study

Fellowship Structure

Pre-Start Shadowing
Final-stage candidates will complete structured shadowing to ensure mutual fit and clarity of expectations.

Month 1

  • 60-minute new patient evaluations under direct observation
  • Intensive supervision and note review
  • Structured psychiatric interview training
  • 4 - 5 patients per day
  • 3 hours weekly didactics (core psychiatry + clinical trials)

Months 2–6

  • Increased autonomy with supervisory oversight
  • Lab ordering and interpretation
  • Expanded follow-up panel
  • ADHD and mood disorder evaluation intensification

Months 7–9

  • Complex case exposure
  • Independent formulation prior to review
  • Bipolar spectrum and personality disorder mastery
  • Rational polypharmacy reduction strategies

Months 10–12

  • Near-independent outpatient panel
  • Weekly formal supervision
  • Scholarly case or literature presentation

Core Clinical Competencies Developed

By completion, fellows will demonstrate mastery in:

  • Evidence-based ADHD assessment (ASRS, WURS, impairment evaluation)
  • Longitudinal stimulant management and risk mitigation
  • Differentiating unipolar depression from bipolar spectrum disorders
  • Treatment-resistant depression algorithms
  • Borderline personality characterization and outpatient boundary management
  • Suicide risk assessment
  • Rational deprescribing and polypharmacy reduction
  • Conservative, evidence-based controlled medication prescribing
  • Lab interpretation and medical monitoring in psychiatric patients

Integrative Psychiatry Training Model

Fellows train within our four-pillar framework:

1. Advanced Psychopharmacology
Diagnostic precision, medication selection, deprescribing strategy, and documentation standards.

2. Integrated Psychotherapy
Use of CBT, ACT, DBT-informed approaches within 30-minute medication visits.

3. Psychiatry–Medicine Interface
Collaboration on:

  • GLP-1 medications and psychiatric considerations
  • Obesity and metabolic syndrome
  • Thyroid disorders
  • Obstructive sleep apnea
  • Restless legs syndrome
  • Cardiac screening
  • Laboratory interpretation and monitoring

4. Lifestyle & Whole-Health Psychiatry
Sleep optimization, exercise, nutritional psychiatry fundamentals, mindfulness principles, and care coordination.

Compensation

Year 1 (Fellowship Training Year):
$80,000 annual salary (W-2)
Protected supervision and didactic time included
No productivity requirement

Year 2 (Post-Fellowship Opportunity):
$110,000 base salary + $20,000 productivity-based compensation
Structured transition to full outpatient panel
Leadership and mentorship pathways available

Year 3 (Post-Fellowship Opportunity):
$110,000 base salary + $30,000 productivity-based compensation
Structured transition to full outpatient panel
Leadership and mentorship pathways available

Qualifications
Required:

  • PMHNP-BC or Physician Assistant (with psychiatric experience)
  • Eligible for Florida licensure
  • DEA license
  • Commitment to outpatient psychiatry

Preferred:

  • Strong interest in evidence-based medicine
  • Comfort with feedback and structured supervision
  • Long-term interest in integrative, physician-led private practice
