Our OUTREACH Case Management service program is growing, and we are looking for 2.0 FTE full-time case managers who want to be part of our growth!
Outreach Case Manager is responsible for providing comprehensive case management services to individuals and families in need. The role involves outreach in the community, assessing client needs, developing and implementing service plans, and connecting clients with appropriate resources and services.
Minimum Qualifications/Requirements
· High School Diploma
· Ability to learn and understand client needs and mental health recovery related to the provision of community mental health services
· Working knowledge of harm reduction and housing first, along with experience working with individuals experiencing homelessness
· Strong written and verbal communication skills, computer skills, and the ability to complete paperwork accurately and in a timely manner
· Meets Licensing requirements of the state
· Valid driver’s license and driving record that meets insurability requirements as defined by the Company’s insurance carrier.
· Complete mandated reporter training, includes training on vulnerable adult law
· Pass a criminal background check
· Be knowledgeable of local housing resources
· Complete mandated Housing Consultation Training
· Complete mandated Housing Transition and Housing Sustaining training
· Ability to read and write English.
· Peer Recovery Specialist (preferred)
Responsibilities
· Provide PATH services such as outreach, screening, case management, re-engagement, to PATH eligible/enrolled clients.
· Perform comprehensive assessments to determine clients’ needs and eligibility for services
· Provide highly mobile street outreach to those experiencing homelessness; including shelters, day drop-in’s, encampments, etc.
· Provide support, assistance, and empowerment to individuals to focus on their recovery
· Demonstrate commitment to SAMSHA's definition of recovery and recovery principles: "A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential."
· Develop individualized service plans in collaboration with clients, outlining goals and steps to achieve them.
· Refer clients to appropriate chemical dependency treatment programs and other necessary resources.
· Provide on-going case management, including regular follow-up and reassessment of clients’ progress.
· Treat all clients with dignity and respect, providing services that are respectful of the person’s culture, age, diagnosis, gender, sexual orientation, religion
· Employ a holistic and culturally appropriate approach when interacting with clients and their interdisciplinary team including promoting health and wellness
· Coordinate integrated services and supports necessary to assist each client as they work towards their goal of recovery.
· Partner with clients to develop an individualized care plan that identifies services and supports that will enable each client to live as independently in the community as possible.
· Problem-solving a range of barriers to mental health and substance use disorder stability
· Complete required paperwork as defined in Minnesota Statutes including case note
· Participate in professional meetings and trainings
· Complete all paperwork in an appropriate, accurate and timely manner and according to agency guidelines.
· Attend all continuing education and training, as mandated by the agency or licensor.
