Job Title: Outreach Case Manager

The Organization:
Homeward Bound is the leading nonprofit organization in Buncombe County focused on preventing and ending homelessness using the best practice Housing First model. We work collaboratively with community partners and have a proven track record of helping individuals and families achieve housing stability. Since 2006, Homeward Bound has moved more than 2,615 people off the streets, out of cars, camps, and shelters and into permanent housing.
Overview: Homeward Bound’s outreach team works to connect homeless individuals to housing and community resources. The Outreach Case Manager is a source of support and guidance to individuals, trained to assist individuals with identifying barriers to housing and engage in goal planning that aims to link individuals to the resources and skills that they need in order to achieve long-term housing. Most importantly, we are looking for individuals that have passion for the work and compassion for all people experiencing homelessness in our community, no exceptions.
This Outreach Case Manager is primarily focused downtown and serves as a liaison between people experiencing homelessness, faith communities, businesses, residents, and City of Asheville emergency services staff, offering harm reduction strategies and problem-solving support to all parties.
Schedule and compensation: This is a full-time, non-exempt position with $24.15 hourly compensation. The schedule has some flexibility but does require some evening and Saturday hours.
Education/Experience Requirements:
BA/BS in Human Services or comparable experience
Prior experience with homeless population or populations with high clinical needs
Demonstrated ability to work independently and proactively

Responsibilities:
Client Services
Proactively outreach individuals experiencing homelessness in Asheville, with a particular focus on downtown
Complete intake paperwork and assessments in order to connect clients with housing resources
Connect clients with supportive services in order to meet both their immediate needs and long-term goals
Work with each client to develop and/or strengthen their goal plan according to individual goals and objectives
Ensure that client interactions are person-centered, goal-focused and reflect each client’s goal plan
Maintain contact with all clients on a scheduled basis that is appropriate to their particular status and needs
Ensure that all services are offered with a trauma informed approach
Arrange and/or provide transportation to clients when required
Exercise patience, understanding, and concern for each client’s well-being, safety, development, and enjoyment of life
Attend weekly case management meetings and other specified meetings/trainings as required
Community
Build and maintain relationships with community mental health treatment providers, crisis service agencies and other community partners working with homeless individuals
Work closely with police and fire departments and other emergency services staff to improve outcomes
Be responsive to community stakeholders questions and concerns about interactions with individuals experiencing homeless
Assist community stakeholders in addressing behavioral issues with individuals experiencing homelessness
Conduct trainings/meetings for community stakeholders about how to best work with homeless community and problem solve any issues
Admin
Ensure all documentation is completed in a timely manner, including data for HMIS
Follow Homeward Bound’s Client confidentiality policy and procedures for sensitive client information.
Assist Coordinated Entry with Data Management and City By Name List referrals
Work with supervisor to complete reporting requirements for county, city, or public grants
