Full-Time | Salaried | Hybrid Reports To: Operations Director Minimum Three (3) In-Person Days Per Week Required

Position Summary

The Outreach Program Manager serves as the operational lead for Community Health Workers (CHWs) and all outreach-based workforce execution across CalAIM, Healthcare Access, STRTP, and ARTGard programming.

This role supervises CHWs, oversees call center assignments, field-based engagement (including door knocking and tabling), mail outreach campaigns, Community Play Dates, and Wellness Labs (CHW-related components), and ensures timely documentation compliance.

The Outreach Program Manager ensures CHWs demonstrate professionalism, complete assigned outreach tasks, and meet documentation and engagement standards aligned with grant, MCP, and organizational expectations.

Clinical supervision of LCMs and therapeutic documentation quality remains under the Clinical Team Lead.

Core ResponsibilitiesCHW Supervision & Workforce Leadership

  • Directly supervise Community Health Workers (CHWs) • Provide regular oversight meetings and performance feedback • Monitor professionalism, reliability, and engagement quality • Ensure CHWs meet outreach and documentation expectations • Address performance concerns in collaboration with Operations Director

Call Center & Outreach Assignment Oversight

  • Assign and monitor CHW call center shifts and outreach call campaigns• Ensure CHWs complete required call quotas and outreach attempts• Monitor documentation of call center engagement activities• Track and verify OTF/RTF documentation related to outreach contacts• Ensure auto-dialer or call tracking systems are utilized appropriately• Provide follow-up for incomplete call assignments or missing documentation

Field Outreach & Community Engagement Standards

  • Ensure CHWs demonstrate professionalism during door knocking, tabling events, and in-person outreach• Monitor adherence to safety protocols and community engagement guidelines• Oversee coordination of mail outreach campaigns and documentation of outreach attempts• Ensure community partner interactions reflect organizational standards• Provide day-of oversight for Community Play Dates, Wellness Labs, and major outreach initiatives

Workforce Scheduling & Payroll Coordination

  • Draft expected shift assignments for outreach events, call center coverage, door knocking initiatives, and community programming• Submit finalized shift schedules to HR for entry into Gusto• Coordinate schedule changes with HR to ensure accurate payroll processing• Monitor staffing coverage to prevent operational gaps

Final payroll processing and compensation authority remain under HR and executive leadership.

Documentation & CalAIM Compliance Oversight

  • Monitor CHW progress note submission timelines• Track OTF/RTF completion across call center, door knocking, and event-based outreach• Maintain documentation compliance dashboards• Provide reminders and corrective follow-up for missing documentation• Escalate repeated non-compliance concerns to Operations Director• Collaborate with Data, Grants & Development Director when documentation gaps impact billing or reporting

Clinical documentation quality remains under Clinical Team Lead oversight.

Outreach Playbooks & Systems Development

  • Lead development and refinement of outreach playbooks for call center operations, field engagement, and tabling• Standardize workflows for documentation, outreach tracking, and event execution• Ensure playbooks align with CalAIM, MCP, and grant compliance requirements

Event Materials, Logistics & Expense Oversight

  • Submit flyer requests and coordinate with marketing/design• Ensure outreach materials align with program guidelines• Coordinate supply inventory and event preparation• Create and submit expense reports related to outreach activities• Ensure expenditures align with grant documentation standards

CHW Training Academy & Proficiency

  • Oversee implementation of the CHW Training Academy• Monitor CHW training completion and competency benchmarks• Ensure CHWs demonstrate proficiency in outreach engagement, call center etiquette, documentation standards, and community professionalism • Coordinate refresher training when needed

Cross-Functional Collaboration

  • Collaborate with Operations Director to ensure smooth day-to-day workforce execution • Participate in weekly 30-minute Revenue & Operations Alignment meeting • Coordinate with Clinical Team Lead when outreach intersects with behavioral health considerations • Work with Data, Grants & Development Director to address compliance or reporting impacts

Qualifications

  • Bachelor’s degree required (Public Health, Social Work, Healthcare Administration, or related field preferred) • 3–5 years experience in outreach, program management, or community engagement • Experience supervising CHWs or community-based workforce preferred • Knowledge of CalAIM, Medi-Cal documentation standards, or grant-funded programs preferred • Strong leadership, organizational, and accountability skills • Ability to manage workforce compliance and field execution simultaneously

Work Environment & Compensation

  • Full-time (40 hours per week)• Hybrid role with minimum three (3) in-person days required. South LA• Additional in-person attendance required for major outreach events• Salary Range: $66,560 – $87,360 annually• Benefits include healthcare benefits, PTO, and professional development opportunities
