Job Summary
The Outreach Worker (Home Visitor) provides family support, case management, and community outreach services to prenatal and postpartum individuals and families with children from birth to age six. This position uses evidence-based home visiting models to promote healthy child development, parenting skills, and family stability. The Outreach Worker collaborates with local health and human service providers to connect families with needed resources and support.
Responsibilities
- Provide home visiting services including screening, assessment, parent education, and family coaching
- Deliver family support and case management to families experiencing housing instability, mental health concerns, poverty, substance use, or other stressors
- Promote early childhood development, safe parenting practices, and family resilience
- Conduct child development screenings and developmental monitoring
- Develop and support individualized, culturally responsive family service plans
- Coordinate referrals and collaborate with community partners, healthcare providers, and social service agencies
- Participate in multidisciplinary team meetings and coordinated care planning
- Complete required training and certification in evidence-based home visiting programs
- Maintain accurate documentation and data reporting in compliance with internal and State requirements
- Protect client confidentiality and maintain professional boundaries
Qualifications
Required (Bachelor’s Level):
- Bachelor’s degree in social work, psychology, early childhood education, or a related field (or equivalent experience)
- Experience working with families, children, or vulnerable populations
- Knowledge of family-centered, strengths-based practice and community resources
Preferred (Master’s Level):
- Master’s degree in social work or related field
- Clinical licensure or working toward licensure

Pay: $56,700.00 - $59,500.00 per year
