PACE Licensed Practical Nurse

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday- Friday 8:00am-4:30pm with on-call rotation

Position Summary:
The PACE Licensed Practical Nurse (LPN) provides direct nursing care and supportive clinical services to participants across the PACE Center, home, hospital, nursing home, and assisted living settings. Working under the supervision of the Registered Nurse and PACE leadership, this role supports implementation of participants’ plans of care through medication administration, observation and reporting of changes in condition, documentation, care coordination, and communication with the interdisciplinary team.

The PACE LPN helps promote participant safety, continuity of care, regulatory compliance, and high-quality care experience for participants and their caregivers.

Position Qualifications:
  • Graduate of an accredited school of nursing or vocational nurse program.
  • Current LPN licensure through the State Board of Nursing is required.
  • Must have at least one year of experience working with the frail and elderly.
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to physically access homes, hospitals, nursing homes, and assisted living facilities, including navigating stairs, narrow spaces, and environments without air conditioning.
  • Strong communication skills, including face-to-face, telephone, and computer skills.
  • Ability to interact successfully with participants, families, and other agencies to present a positive and professional image.
  • Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
  • Knowledge of the principles and practice of primary medical care.
