PACE Transportation Manager

Title: Transportation Manager
Location: Lafayette, CO
Schedule: Monday-Friday, 8am to 5pm
Pay: $72,719 to $90,925 per year
JOB SUMMARY
The Transportation Manager is responsible for the coordination and implementation of transportation services within the PACE program, including ensuring transportation to and from appointments and handoffs during arrival and departure at the PACE center are conducted safely and efficiently. Ensuring coordination of care between PACE and contracted facilities, transportation vendors, and providers. Responsible for departmental compliance with state and federal regulations, participation in organizational Quality Assurance program, departmental training, education, and coordination of provision of program services as part of the PACE IDT leadership structure.
RESPONSIBILITIES
  • Oversees a team of transportation specialists (Drivers), transportation daily schedules, payroll, and performance management
  • Communicate IDT decisions, participant changes, care plan changes, and updated information to transportation staff and receptionist / administrative assistant daily.
  • Serve as liaison with transportation vendor for all matters pertaining to transportation, including problems, schedule changes, delivery of equipment and other items, and reconciling invoices from vendor(s).
  • Ensure responsibility for running and maintaining transportation scheduling software program.
  • Coordinate and schedule the daily delivery of home delivered meals, DME, supplies and miscellaneous items as requested by the IDT team.
  • Oversee vehicle maintenance program and obtain estimates and authorize vehicle repairs per departmental policies.
  • Ensure that vehicles have up-to-date Department of Motor Vehicle registration forms, license plates and emission stickers.
  • Ensure cleanliness and proper functioning equipment as well as safe driving conditions for each vehicle.
  • Demonstrates effective leadership in managing daily department operations, managing staff, planning, and evaluating department activities to accomplish department objectives.
  • Interviews, hires, trains, coaches, and provides constructive feedback and training opportunities to employees. Conducts employee disciplinary action in collaboration with supervisor and HR as needed.
  • Develops annual department and individual employee goals, assesses employee performance, and develops employee performance improvement plans in conjunction with HR when needed.
  • Participates as an active member of PACE IDT and TRU Community Care Leadership groups.
  • Maintains professional affiliations and any required certifications.
EDUCATION/EXPERIENCE
  • 2-3 years relevant experience working as a transportation manager
  • Associate Degree in Health or Human Services, Business or another related field. (May substitute CNA License for degree requirements).
  • Experience with transportation scheduling computer software, and working in a regulatory environment.
  • Experience working with individuals with mental health needs (Preferred).
  • Experience working with the frail or elderly population (Preferred).
  • Experience in team management, preferably in a geriatric care setting.
  • Experience working within an Interdisciplinary Team Model.
