PACT Care Coordinator - Case Manager

Bridgemont Health and Wellness is recruiting for a full-time Care Coordinator to work on our PACT team (Program for Assertive Community Treatment.) The PACT team is an interdisciplinary team which works with people who have been diagnosed with a severe, persistent mental illness. Duties include: assisting clients with adult daily living skills and providing linkage and referral as needed. Schedule is 9am - 6pm, ROTATES NIGHTTIME, WEEKEND AND HOLIDAY EMERGENCY CALLS WITH OTHER CLINICAL PACT STAFF.

Prefer Bachelor’s degree in a human services field with at least one year of full-time experience in a mental health setting. Requires excellent verbal and written communication skills, with ability to do detailed paperwork utilizing an electronic medical records system. Must have a valid driver’s license and a working knowledge of community services.

Job Type: Full-time

Pay: $42,848.00 - $43,704.96 per year
