PACT Case Manager
Our Program for Assertive Community Treatment (PACT) Team is seeking an experienced Case Manager to join their team. The PACT Case Manager is a non-exempt position under the supervision of the PACT Program Supervisor. The PACT Case Manager is responsible for engaging with PACT Program clients, med-monitoring, and other clinical duties.
This is a full-time position.
The starting wage for this position is $24.20/hr for a minimally qualified candidate - Additional years of applicable experience will increase the starting wage offered.
Minimum Qualifications
Preferably Bachelor’s Degree in behavioral sciences or related field, or Associate of Arts with at least two (2) years’ experience in mental health or related field working with serious and persistent mentally ill adults.
Good organizational skills.
Ability to work independently and manage multiple tasks.
Ability to work cooperatively in a team approach.
Ability to apply a Recovery based approach and to apply clinical knowledge.
Solid verbal and written communication skills, basic computer skills.
Ability to exercise independent judgment and discretion.
Visual and auditory skills required to communicate, ensuring proper client care, and to provide legal documentation of care.
Requires frequent standing, bending at the knees and waist, kneeling and reaching; Requires frequent walking to accomplish tasks; Requires ability to lift 30 pounds.
Employment contingent on successful background check and Washington State counseling profession credential issued by the Department of Health (e.g. Agency Affiliated Counselor).
Job Duties:
· Participate in client-centered comprehensive assessment
· Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
· Consult with community agencies and families to maintain coordination in the treatment process
· Participate in daily staff organizational meetings and treatment planning review meetings
· Provide ongoing assessment, problem-solving, side-by-side services, skill training, supervision, and environmental adaptations to assist clients with activities of daily living Other job duties as assigned.
