Palliative Care Coordinator
Summary of Responsibilities:
The Palliative Care Coordinator plays a pivotal role in facilitating compassionate and timely access to palliative care services. As a crucial member of the interdisciplinary team, this professional ensures seamless communication among patients, families, healthcare providers, and the palliative care team, by managing the referral process, coordinating appointments, and maintaining accurate documentation. While demonstrating sensitivity, empathy, and professionalism in all interactions, respecting diverse backgrounds and emotional states.
Reports To: Director of Clinical Services
Responsibilities:
  • Serves as the primary point of contact for palliative care referrals, receiving and processing requests from physicians, nurses, social workers, or other healthcare professionals.
  • Verify referral information, collect relevant patient data, and initiate the referral workflow in accordance with policies & procedures and regulatory requirements.
  • Communicate with patients and families to introduce them to available services, answer basic questions about the palliative care process, and provide logistical support related to appointments and follow-up.
  • Schedule initial and follow-up appointments with palliative care providers, ensuring that all necessary medical records, consents, and background information are available at the time of the visit.
  • Collect, organize, and maintain accurate and up-to-date patient records related to palliative consultations and services, including histories, test results, advanced directives, and care plans. Manage sensitive information in strict compliance with privacy and confidentiality regulations (e.g., HIPAA).
  • Coordinate and participate in palliative care interdisciplinary group meetings (PC IDG), and case conferences, to relay updates, share observations, and clarify logistical details as warranted.
  • Assist with patient intake, vitals, and symptom assessments for palliative care visits (in-person or telehealth) as needed.
  • Document patient interactions accurately in the electronic medical record (EMR).
  • Support care coordination across the continuum of healthcare and with referral sources.
  • May make home visits if needed.
  • Manage the call list to follow up with cancelled referrals and monthly follow up calls.
  • Accurately document call list in accordance with the established acuity scale to determine call/follow-up needs.
  • Educate patients and families on available resources, both within the healthcare system and in the broader community, such as home health services, durable medical equipment, support groups, and hospice services.
  • Manage phone calls, correspondence, and electronic medical records (EMR).
  • Track referral metrics and generate reports for quality improvement initiatives.
  • Maintain a thorough understanding of palliative care guidelines, eligibility criteria, and service standards.
  • Maintain a thorough understanding of hospice services and eligibility criteria to educate patients with qualifying options and needs.
  • Ensure that referrals and documentation meet all regulatory, insurance, and organizational requirements.
  • Provides back up to the hospice referral team when necessary.
  • Coordinate time off needs with direct supervisor and the hospice referral team to ensure referral coverage.
Qualifications and Skills:
  • High school diploma or equivalent, required.
  • Prior experience in a medical office, hospital, or palliative care setting, required.
  • Proficiency with EMR, referral management software, Microsoft applications to include strong proficiency in Excel, desired.
  • Exceptional organizational, time management, and multitasking abilities.
  • Demonstrates accuracy in documentation, appointment scheduling, and information relay.
  • Strong interpersonal and communication skills, with an emphasis on compassionate care and cultural sensitivity.
  • Ability to remain calm, professional, and empathetic.
  • Confidentiality, discretion, and an understanding of privacy laws and ethical standards in healthcare.
  • Ability to adjust efficiently to changing priorities, urgent needs, and complex care coordination scenarios.
  • Conveys information clearly to both clinical and non-clinical team members.
  • Proactively identifies barriers in the referral process, collaborating with direct supervisor to implement appropriate solutions.
Schedule:
This is a part-time position working on-site 24 hours per week. Schedule to be determined. This position is non-exempt under the Fair Labor Standards Act.
