Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed symptom evaluations to develop personalized care plans.
- Manage complex pain and symptom control using advanced pharmacological and non-pharmacological interventions, including acute pain management techniques.
- Collaborate with multidisciplinary teams to coordinate care, discharge planning, and facilitate communication among patients, families, and healthcare providers.
- Administer medications and treatments such as IV infusions, botulinum toxin therapy, dermal fillers, and perform procedures like suturing and catheterization with precision.
- Utilize electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks to document patient care accurately while maintaining HIPAA compliance.
- Provide telehealth consultations to extend palliative services remotely, ensuring access for home-bound or remote patients.
- Educate patients and families on disease processes, medication administration, nutrition, and advanced directives to promote informed decision-making.
Experience
- Valid Nurse Practitioner license with certification in hospice & palliative medicine or related specialty.
- Demonstrated experience in critical care settings such as ICU or Level I Trauma Centers, with exposure to emergency medicine or hospital medicine preferred.
- Proficiency in managing complex cases involving infectious disease care, geriatrics, behavioral health, or pediatric palliative needs including neonatal ventilator management or Toddler care.
- Hands-on experience with medical procedures such as spinal taps, tube feeding management, venipuncture, sterile processing techniques, and airway management.
- Familiarity with medical coding systems including CPT, ICD-9/10/ICD coding for documentation accuracy; experience with billing for services like Botox treatments or esthetic laser procedures is a plus.
- Strong knowledge of physiology and anatomy relevant to palliative care scenarios; ability to perform diagnostic evaluations using sonography or laboratory testing when needed.
- Experience working within diverse healthcare environments such as assisted living facilities, nursing homes, outpatient clinics, or post-acute care settings; previous telehealth experience is advantageous.
