We are expanding aggressively and have recently added 300+ patients across 70+ group homes in the Valley, creating an immediate need for a strong, independent provider.
The Opportunity
We are seeking a full-time Palliative Care Nurse Practitioner to own and manage a dedicated panel of patients in Scottsdale, Mesa, Gilbert, and surrounding East Valley areas.
This is NOT a slow ramp role. You will step into an existing patient population immediately with strong organizational support behind you.

What You’ll Do
- Provide in-home palliative care visits in group homes, assisted living, and private residences
- Manage patients with advanced chronic illness and serious medical conditions
- Conduct:
- Symptom management
- Goals of care discussions
- Medication management
- Care coordination with families, caregivers, and facilities
- Collaborate with our:
- Primary care team
- Transitional care program
- Hospice partners when appropriate
- Maintain accurate documentation in eClinicalWorks (eCW)
- Participate in a high-performing, fast-paced care model
Patient Profile
- Primarily Medicare population (~80%)
- High-acuity, multi-comorbid conditions
- Mix of:
- Group home residents
- Assisted living patients
- Homebound individuals in private homes
Compensation
- Productivity-based compensation (no cap)
- Strong earning potential with an immediate patient panel
- Mileage reimbursement
- Malpractice coverage
- Flexible scheduling with autonomy
(This is a high-output role with systems in place to support strong earnings from day one.)

Requirements
- Active Arizona NP license
- Board-certified Nurse Practitioner (FNP or AGNP)
- Experience in palliative care, hospice, geriatrics, or home-based care strongly preferred
- Comfort managing complex, high-risk patients independently
- Strong communication and documentation skills
- Ability to travel throughout the East Valley
