Overview
Supportive Care Specialist Group is seeking a compassionate and experienced Palliative Care Nurse Practitioner NP to provide patient-centered care in Brea/La Mirada, CA across hospital and LTAC settings.

This role offers the opportunity to practice with strong clinical autonomy while working within a collaborative interdisciplinary team. You will care for adult and geriatric patients with complex conditions and support them and their families through important care decisions.

Key Responsibilities

• Conduct daily patient rounding in hospital and LTAC settings
• Provide palliative care consultations and symptom management
• Lead goals of care discussions with patients and families
• Collaborate with physicians nurses and interdisciplinary teams
• Develop patient centered care plans
• Maintain timely and accurate clinical documentation

Qualifications

Required

• Active California Nurse Practitioner NP license
• Minimum 2 years inpatient hospital experience
• Minimum 2 years primary care experience
• Strong communication skills and compassionate bedside manner

Preferred

• Experience in palliative care hospice or geriatrics
• Bilingual Spanish speaking is a plus
