Destiny Hospice and Palliative Care is looking for qualified LVNs to join our Palliative Team!

Full-time Day Shift 8:30 am-5:00 pm

Territories- Moreno Valley Office / Inland Empire

Your Role as a Licensed Vocational Nurse

• Communicates effectively on the telephone with patients, families, health plans, providers and staff.
• Manages Cases assigned by Clinical Supervisor/Nursing Director and assumes responsibility for symptom management and coordinating of care for assigned caseload.
• Communicates with the physician, heath plans and other palliative care team regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Orders and maintains accurate records of medical equipment and supplies.
• Responsible for performing patients’ visit as needed and assisit with specialty care .
• Participates in personal, and professional growth and development. Also participates in IDT and QAPI activities.
• Performs other duties as assigned by the Clinical Supervisor/Nursing Director.

Required license or certification: CA Licensed Vocational Nurse (LVN/LPN), CPR

Salary Range- $27.00-35.00

Pay: $27.00 - $35.00 per hour
