PALLIATIVE/HOSPICE APN - PART-TIME 16-24 HOURS/WEEK

The Palliative/Hospice Advanced Practice Nurse (APN) will act as a leader in disease-modifying care, pain, and symptom management, giving attention to the highest quality of life possible, and compassionate end-of-life-care. The APN will represent the Campus in activities involving professional contacts with patients/families, physicians, hospitals/facilities, senior living communities, professional associations and similar health groups and institutions.

Duties, responsibilities, and activities may change, or new ones may be assigned at any time with or without notice. Responsibilities include, but are not limited to:

  • Conducts comprehensive, focused assessments of patients and families with palliative care needs.
  • Work with patients and families to determine their understanding of the patient’s condition, proposed treatment plan and advance care planning.
  • Formulates, implements, monitors, evaluates, and modifies palliative care plans with input from an interdisciplinary palliative care team and other collaborating care providers (primary care and specialists).
  • Coordinates care with social workers, case managers and applicable ancillary staff regarding hospice and palliative care services.
  • Assist patients and families in developing treatment plans, including the management of medications and pain control.
  • Assist in all facets of care coordination for palliative care referrals.
  • Completes advance care planning with patient and family and refers to hospice as is appropriate.
  • Engage in quality improvement in patient care services and department functions, including evaluating the effectiveness of the health care delivery system in meeting the health care needs of palliative care patients and families.
  • Plans clinical and educational interventions, which are consistent with individual/organizational needs and goals and evaluate change/outcomes.
  • Partners in the development and growth of the Palliative Care Program.
  • Participates in palliative care research with a focus on outcomes.
  • Prepares and maintains accurate patient records, charts, and documents to support sound hospice and palliative practices and reimbursement for services provided. Assists in the development of clinical documentation protocols.

Experience:

  • APN: 1 year (Required)
  • PALLIATIVE CARE: 1 year (Required)

License/Certification:

  • NEW JERSEY RN LICENSE (Required)
  • ACCREDITED NURSE PRACTITIONER (Required)
  • DEA LICENSE (Required)
  • NEW JERSEY DRIVER'S LICENSE (Required)
