Part Time Family Nurse Practitioner

About the Role
We are seeking a compassionate and experienced Family Nurse Practitioner to join our nationally recognized Patient-Centered Medical Home on a part-time basis. This role primarily focuses on Chronic Care Management (CCM) for adult patients with conditions such as diabetes, hypertension, and asthma, while supporting primary care services in an outpatient setting.

Key Responsibilities

· Manage and monitor care plans for patients with chronic medical conditions

· Perform patient evaluations, physical exams, and health assessments

· Diagnose and treat acute and chronic illnesses

· Order and interpret labs and diagnostic tests

· Educate patients on disease management and lifestyle modifications

· Collaborate with physicians and clinical staff to coordinate patient care

· Refer patients to specialists when appropriate
