Part-Time Medical Social Worker (MSW) – Home Health

Job Overview

Nurses Care Inc. is seeking a Part-Time Medical Social Worker (MSW) to join our Home Health team. The Medical Social Worker provides psychosocial assessments, care coordination, and resource support to patients receiving skilled services in their homes.

This role is ideal for a licensed social worker with healthcare, home health, hospice, hospital, or case management experience who is passionate about helping patients and families navigate healthcare challenges

Responsibilities

· Conduct psychosocial assessments for home health patients

· Provide emotional support and counseling to patients and caregivers

· Identify social determinants of health impacting patient care

· Connect patients with community resources, financial assistance, and support services

· Collaborate with nurses, therapists, physicians, and care teams

· Assist with care planning and discharge planning

· Provide case management and patient advocacy

· Complete accurate and timely documentation in accordance with home health regulations

Qualifications

· Master’s Degree in Social Work (MSW) from an accredited program

· Current Social Work license in Ohio (LMSW, LCSW, or equivalent if required)

· Home Health, Hospice, Hospital, or Healthcare Social Work experience preferred

· Strong knowledge of community resources and care coordination

· Ability to work independently in patient homes

· Valid driver’s license and reliable transportation

Pay: $60.00 per hour
