Job Summary
Provides direct primary care health services to patients ages newborn- 21 years working independently under the Medical Director's Collaborative Practice Agreement. this school linked adolescent health center is designed to provide comprehensive primary care, psychosocial and mental health services, health promotion/disease prevention, and outreach services.
Essential job duties:

Comprehensive well child and adult assessments.

Immunization assessment and administration using the Michigan Care Improvement Registry (MCIR)

Minor injury assessment and treatment
-
Assessment and treatment of acute and chronic health conditions.

STI, HIV, and Reproductive Health Services, not including abortion services or referrals
-
Primary Care, including health maintenance/promotion and health education for patients and family members and preventative care

Hearing and vision screening
-
Laboratory services - order, perform, interpret

Urgent or emergent care as indicated

Telehealth visits
-
Confidential/minor consented services as allowed by state and/or federal law
Additional Responsibilities

Work as part of an integrated care team that includes: Medical Director, medical staff, Nurse Care Coordinators, Medical Assistants, Behavioral Health Providers, Care Manager, Patient Service Representatives, and Program Director

Perform office management activities, including supervision of any on-site clinical, mental health and administrative staff and related supervisory duties; ordering supplies, drugs, vaccines, etc,; inventory control; other essential functions to ensure the clinic is effectively managed and operationally run.

Conduct client satisfaction surveys

Oversee the local community advisory council
-
Provide routine nursing/medical assistant care as needed

Liaise with the Executive Office Coordinator to coordinate purchasing activities, manage supply orders, and maintain accurate inventory levels.

Review MA and nurse visits/encounters

Complete timely accurate documentation and billing activities in electronic health record

Understands social determinants of health and trauma-informed care
-
Provide culturally competent care
-
Attend daily staff huddle, weekly clinic check-in, monthly staff meetings and scheduled program meetings with state advisors.

Serve on Quality Improvement (QI) Committee and participates in other Health Center committees as needed.

Complete routine compliance trainings and activities as directed by Compliance Officer, uphold all policies and procedures

Ensure compliance with health and safety standards, infection control, and quality assurance processes

Participate in Medicaid outreach and community outreach activities
-
Carries out other duties as requested by the Clinic Manager and Program Director
Location
Required Education, Experience, and Licensure:
State of Michigan Nurse Practitioner License (FNP, PNP)
Minimum of two years of experience in the field of Family Medicine
DEA License
Working Conditions/Special Conditions
Patient care environment; frequent exposure to body fluids, infectious disease, and potentially hazardous chemicals.
