The
Patient Care Coordinator
is a key administrative and front-office partner who creates a welcoming, organized, and supportive experience for patients and families from the moment they enter our clinics. Serving as the first point of contact, this role plays a critical part in ensuring smooth clinic operations, clear communication, and a positive patient experience.

This is a part-time position that will be around 25 hrs/week (can be flexible). We need someone who is available evening weeknights until 8pm.
The Patient Care Coordinator works closely with clinicians and care teams to support scheduling, registration, documentation, and day-to-day clinic flow. This role is ideal for someone who is detail-oriented, service-driven, and energized by supporting both people and processes in a fast-paced healthcare environment.
WHAT YOU’LL DO:

Serve as the primary front desk point of contact, creating a welcoming and professional experience for patients and families

Manage check-in, registration, and scheduling workflows, ensuring accuracy and efficiency

Collect, verify, and maintain patient demographic and insurance information in the EMR

Handle administrative tasks including phone calls, documentation, scheduling requests, and visit preparation

Communicate and collaborate with clinicians and clinic staff to support smooth clinic operations

Support patient outreach and follow-up related to scheduling and care coordination needs

Maintain a clean, organized front-office environment and assist with opening and closing routines

Provide cross-coverage support, assist with training, and contribute to process improvements as needed
WHAT WE LOOK FOR:

High school diploma or GED required; additional education or equivalent experience is a plus

1+ year of experience in an administrative, front desk, customer service, or patient-facing role

Comfort using computer systems and scheduling platforms (e.g., Microsoft Office, EMR or similar systems)

Healthcare experience preferred

Strong organizational skills with the ability to manage multiple priorities and maintain attention to detail

Clear, professional, and empathetic communication skills—both written and verbal

Ability to remain calm, flexible, and collaborative in a fast-paced environment

Experience working with children and families is highly valued

Salary ranges may vary based on geographic location, local market conditions, state-specific factors, and individual qualifications, including years of experience, education, and scope of responsibility.
