Job Title: PEDIATRIC RN CASE MANAGER/QA POSITION SCOPE:

The Pediatric RN Case Manager is an integral part of the care team ensuring the smooth and consistent delivery of home care services through administrative oversight, and adherence to regulatory practice and industry standards. The successful candidate will have a broad background in the delivery of home health care services, be knowledgeable in documentation and completing OASIS and preparing 485’s and submitting documentation for the authorization of services. It is essential to have exceptional communication skills with the ability to maintain, develop and foster relationships with staff members, patients and their families, as well as with doctors and staff at insurance companies. Reliable transportation is a must as this position requires a considerable amount of travel throughout Delaware, Montgomery, Philadelphia and Bucks counties.

This position reports to Clinical Manager and Administrator.

RESPONSIBILITIES: Assists in the oversight of clinical activities Assists in conducting case conference Orients new staff, Supervises nursing and home health aide staff Assists in chart reviews and reconciliation and QA/QI activities Prepares Plan of Cares and physician orders Investigates and resolves concerns and incidents

POSITION QUALIFICATIONS: Preferred 2 years supervisory experience in home health care, pediatrics

Pediatric experience - preferred

Vent/trach and parenteral and enteral nutrition a plus Working knowledge of OASIS C Graduate of an accredited School of Nursing Licensed in the State of Pennsylvania Professional Liability Insurance with $1,000,000/3,000,000 coverage Clear driver's license and use of automobile. Automobile Insurance with $1,000,000/3,000,000 coverage. - Optional

Good verbal and written skills required
