Job Title: PRN /Part-Time Field LVN – Home Health

Schedule: Monday-Friday (Flexible Schedule)
Compensation: $25.00 – $35.00 per hour (DOE)

Position Summary:
We are seeking a reliable, detail-oriented Licensed Vocational Nurse (LVN) to join our home health team on a part-time basis. The LVN will provide skilled nursing care to patients in their homes, focusing on clinical excellence, safety, and compliance with care plans. This role is ideal for a nurse seeking consistent weekday hours without evening or weekend shifts.

The nurse must possess proven experience in wound VAC management, wound dressing application, and IV dressing changes.

Responsibilities:

• Provide direct patient care in accordance with the physician’s plan of care and agency policies.
• Conduct comprehensive assessments, record observations, and document care accurately in EMR.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease processes, medications, and home care techniques.
• Monitor and report changes in patient condition to the RN Case Manager and/or physician.
• Maintain timely communication with the clinical team to coordinate services.
• Travel to patient homes within the 1604 Loop service area.
• Comply with all infection control, safety, and regulatory standards.

Qualifications:

• Current Texas LVN license in good standing.
• Minimum of 1 year of nursing experience (home health experience preferred).
• Current CPR certification.
• Valid Texas driver’s license, reliable transportation, and proof of auto insurance.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage time effectively.
• Comfortable with electronic charting systems (EMR).
