Position Overview
We are seeking a highly organized, emotionally intelligent, and resilient Patient Care Coordinator (PCC) to join a fast-paced, high-acuity healthcare environment. This is not a routine administrative role. Our PCCs operate at the center of complex patient care coordination, often managing urgent and evolving situations where delays, miscommunication, or lack of attention to detail can directly impact patient outcomes.
The ideal candidate thrives under pressure, communicates confidently with a wide range of stakeholders, and understands how to prioritize competing demands in real time. This role requires professionalism, empathy, urgency, critical thinking, and the ability to remain calm and organized while handling sensitive, time-critical cases.
You will work closely with patients, families, providers, medical directors, case managers, payers, leadership, and multidisciplinary teams to ensure seamless coordination of care and exceptional service delivery. Candidates who are uncomfortable with direct communication, rapid task-switching, accountability, or high-volume coordination environments may not be successful in this role.

Key Responsibilities
Patient Care Coordinators (PCCs) are responsible for managing and coordinating patient care services through our provider network while ensuring exceptional communication, operational accuracy, and patient support throughout the continuum of care.
Responsibilities include, but are not limited to:

Communicating professionally and effectively with patients, families, providers, payers, pharmacies, and internal teams regarding patient care coordination, updates, scheduling, and case progression

Managing high-volume communication across multiple platforms including phone, email, fax, EMR/CRM systems, and internal communication tools

Documenting all patient interactions, care updates, and coordination efforts accurately and in real-time within JIPA’s EMR/CRM systems
-
Maintaining and updating Plans of Care (POCs) and Request for Quotes (RFQs) under guidance from medical directors and case managers
-
Coordinating complex patient cases that may involve urgent changes, escalations, scheduling conflicts, authorization issues, or evolving clinical needs
-
Prioritizing patient cases based on urgency, clinical needs, and operational timelines
-
Scheduling, confirming, monitoring, and following up on appointments, services, deliveries, and provider coordination activities

Ensuring all required documentation, authorizations, consents, medical records, and compliance-related materials are collected, verified, and submitted accurately and on time

Coordinating pharmaceutical, DME, and service-related orders and deliveries for patients
-
Verifying that clinical activities align with treatment plans, operational workflows, and JIPA standards

Identifying and escalating potential risks, delays, confidentiality concerns, clinical issues, or operational barriers to appropriate leadership

Supporting patient retention by building strong, compassionate, and professional patient relationships

Participating actively in patient censuses, departmental meetings, operational reviews, and training sessions

Collaborating closely with peers, leadership, medical directors, and multidisciplinary teams to maintain efficient workflows and high standards of patient care

Adhering to all company policies, workflows, compliance standards, and professional ethics expectations

Providing additional operational and administrative support as needed to maintain continuity of care and workflow efficiency
