Patient Care Coordinator for Surgery Practice

Daily Tasks May Include:

  • Preparing patient charts for upcoming appointments to include secure any documents or images needed from the hospital or referring facility as needed for coordination of care.
  • Obtaining and recording vital signs (height, weight, blood pressure, pulse, respiration and temperature)
  • Updating patient records based on changes to medications and health history between visits
  • Assisting provider and other members of the clinic team in carrying out and documenting next steps in the patient's plans of care
  • Scheduling follow up patient appointments as needed
  • Ordering any tests or medications as requested by the treating provider
  • Processing referrals for therapy, imaging, DME, home care, and/or other providers
  • Responding to patient questions and concerns

Skills/Requirements:

  • One (1) year of experience in a healthcare setting
  • Three (3) years of previous experience as a caregiver, preferred
  • Strong verbal communication skills
  • Comfortable working within multiple software systems including EHR, PM, PACS and Microsoft Office

Pay: From $16.00 per hour
