Job Title: Patient Care Coordinator

Salary Range: Compensation ranges from $21.52 to $30.12, depending on years of experience.

Direct Report Title: Clinic Manager

Summary: Patient care coordinators are liaisons between patients and the healthcare system. As a patient care coordinator, you would ensure that patients receive the care they need and that they understand their medical condition, medications, and other instructions.

Qualification: Requires one year of experience in a clinical position with demonstrated competency in performing skills previous pulmonary experience preferred. Requires successful interpersonal and communication skills with patients, families, physicians, and other health care team members in order to foster optimal quality of outpatient care. Excellent communication (written and verbal) and interpersonal skills; ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians. Proficient computer skills including keyboarding, navigation within a windows operating system, and use of electronic mail with exposure to electronic medical records systems. Medical setting experience preferred

Education: High School Diploma or GED is required.

Patient Care Coordinator Duties and Responsibilities:

  • Plan, coordinate and carry out patient care operations.
  • Address patient requests and needs in a professional manner.
  • Respond to patient concerns timely and ensure patient satisfaction.
  • Schedule patient appointments and make reminder calls.
  • Greet patients and direct them to examination rooms.
  • Maintain patient medical records up-to-date.
  • Obtain insurance information from patients, process patient billings, and collect payments.
  • Collect and maintain patient health, demographic, insurance and financial information.
  • Develop a patient care plan based on Physician’s assessment information.
  • Suggest process improvements to enhance patient services.
  • Maintain patient records confidential and secure.
  • Arranges for Insurance pre-certifications and patient registration prior to patient services.
  • Maintains patient records in an organized manner according to accreditation regulations and departmental protocols.
  • Participates in and contributes to departmental performance improvement activities. Gathers information and prepares reports in the approved format quarterly for HCH PI meetings.
  • Maintains good working relationship with Medical Directors, Physicians, Office Personnel, other hospital department employees and Sleep Lab employees.
  • Maintains patient confidentiality in all settings with sensitivity to privacy concerns of patients.
  • Enters patient charges accurately and in a timely manner.
  • Maintain satisfactory, regular and predictable attendance by reporting to work as scheduled keeping unscheduled absences to a minimum. Determines priorities in workload and completes projects in an accurate and timely manner.
  • Ensure all communications reflect the standards and mission of PPMG.
  • Maintains patient confidentiality and follows HIPAA Guidelines.
  • Display a consistently positive, cooperative, self-motivated, courteous and professional attitude.
  • All other duties assigned.
