Position Summary
We are seeking a highly motivated and results-driven Patient Care Coordinator (PCC) to join our dynamic patient services team at Evolve Telemedicine. As a PCC, you will help patients live happier, healthier, and more productive lives through Evolve’s hormone and peptide treatments. Your primary responsibility will be to provide an outstanding patient experience while supporting Evolve’s providers by coordinating appointments, medications and refills, coupled with patient education about current and new diagnostics and treatments that will improve overall health and wellbeing. You will collaborate closely with Evolve’s patient intake coordinators and sales manager. Keen organization skills, coupled with a commitment to urgency, persistence, and consistency will be crucial. Strong interpersonal skills and the ability to work in a fast-paced environment are essential for success in this role. A passion for health and wellness, and helping others to achieve personal successes is critical.
Duties and Responsibilities
The Patient Care Coordinator duties include but are not limited to:
- Provide outstanding support to Evolve patients on their journey from initial consultation, to lab draws and review, diagnosis, and prescriptions and refills
- Review Evolve lab panel results, consult with providers, and communicate with Evolve patients regarding their care
- Provide education and act as a resource to Evolve patients on hormones and peptides
- Development of proficiency in all medications and products within Evolve's formulary
- Support physicians and advanced practice providers throughout the patient journey
- Provide an outstanding patient experience with proficiency in customer success principles
- Use patient retention techniques to ensure maximum patient retention
- Demonstrate mastery in all aspects of Optixx, including patient profile, estimates, refills, completing tasks, and using reporting to increase patient retention and satisfaction

Position Requirements
