Job Summary
We are seeking a dedicated and compassionate Patient Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate and oversee patient care plans, ensuring quality patient outcomes with our Chirporactic and Physical therapy Team. Responsibilities
Patient Case manager responsibilities include, but are not limited to, the following:
● Fully memorizing all scripts relating to enrollment including:
○ Report of Findings for all services
○ Outbound and Enrollment Calls Scripts
○ Initial consult script
○ Training with their supervisor for minimum 1-2 hours per week
○ Record all Report of Findings and review for continual improvement
○ Outbound calls to pending leads for the office
*The Employee's duties may be reasonably modified at the Company's discretion from time to time
Key Performance Indicators:
- Convert New Patients to Enrollment at 25% or above
- Convert ROF to enrollment at 35% or above
- Create a patient retention rate of 93% or above of enrollments
- Minimum of 20 outbound calls a day with 5 scheduled - 100% Case Manager Follow ups with patient on or by 7th visit

Work environment— This position operates in a high paced medical office, focusing on a niche market, highly respected in our industry. Consultation will be performed face-to-face with patient and supporting family members within the Case Manager’s office.
Physical demands— Able to sit, stand and walk
Position type and expected hours of work—
● PartTime ( with transition to full time) Monday-Wednesday-Friday
Valid driver’s license and reliable transportation required.
Required education and experience— Minimum of 5+ years of previous new business development/sales experience, Customer Service Experience, High school diploma
Preferred education and experience—10+ years in new business development and sales experience.
Pay: $25.14 - $30.27 per hour
