Position Overview

We are seeking a Patient Engagement Coordinator (Diabetes) to serve as the dedicated account owner for patient outreach and remote support efforts at our partner clinics.

In this hybrid role, you will act as the bridge between the clinic and their patients. You are not just "calling a list"—you are taking ownership of a specific customer account’s success. Your primary focus is providing concierge-level administrative care: educating patients on the value of SweetSpot, enrolling them in the program, and proactively troubleshooting technical issues with CGMs and insulin pumps.

You will work closely with our Customer Success and Clinical teams to ensure smooth patient onboarding, drive remote monitoring success, and report back to practice leadership on the status of their patient population.

What You’ll Do

Own the Account & Patient Workflow Take full responsibility for the patient engagement success of your assigned clinic(s). You will manage the entire patient pipeline—from initial outreach and education to enrollment and retention—ensuring the clinic sees high patient adoption rates.

Provide Concierge-Level Administrative Care Act as the primary point of contact for patients, offering proactive, white-glove support. You will check in to ensure everything is going smoothly, answer program questions, and resolve administrative concerns so patients feel supported between doctor visits.

Lead Patient Education & Enrollment Conduct outbound calls to eligible patients to educate them on the benefits of the SweetSpot program. You will explain the "why" behind remote monitoring, obtain consent, and guide them through the enrollment process.

Provide Technical Device Support Help patients set up remote data sharing for their CGM and insulin pumps (e.g., Dexcom, Libre, Tandem, Medtronic). You will troubleshoot syncing issues and walk non-technical patients through app or portal setups with patience and clarity.

Communicate with Practice Stakeholders Serve as the administrative liaison for the clinic. You will document interactions in the CRM/EMR and participate in practice meetings to provide updates on enrollment numbers, patient feedback, and program success to clinic leadership.

You're a great fit for this role if:

  • You have "Back Office" Endo Experience: You’ve worked as a Medical Assistant (MA) or in an administrative support role within an endocrinology office and understand the workflow of a diabetes clinic.
  • You are an EMR Pro: You have experience navigating different EMRs and are comfortable documenting patient interactions in detail.
  • You take ownership: You don’t just complete tasks; you care about the results of the specific client account you manage.
  • You are tech-fluent: You are familiar with platforms like Dexcom Clarity, LibreView, or CareLink and enjoy helping others use them.
  • You are a "People Person": You are confident on the phone, can break down technical instructions into simple language, and provide reassuring, friendly support.

Required Qualifications

  • Support Experience in Diabetes Care: At least 3 years of experience as a Medical Assistant (MA), CNA, or Patient Care Coordinator within an endocrinology clinic or diabetes-focused setting.
  • EMR Proficiency: Demonstrated experience working inside Electronic Medical Records (EMRs) and maintaining accurate patient logs.
  • Device Familiarity: Strong familiarity with diabetes technologies like Dexcom, Freestyle Libre, Eversense, or insulin pumps (e.g., Tandem, Medtronic, Omnipod).

Preferred Qualifications

  • Prior experience in a Customer Success or Account Management role.
  • Experience in telehealth, remote patient monitoring (RPM), or a technology-enabled care setting.
  • Bilingual (Spanish-speaking) is a plus.

Job Type: Full-time

Pay: $40,000.00 - $50,000.00 per year
