Patient Engagement & Installation Specialist (Field Role)

Help seniors transition safely from hospital to home by introducing and guiding them through a Medicare-covered post-discharge program. Independent, field-based role with flexible part-time schedule, mileage reimbursement, and performance bonuses. Ideal for someone confident, assertive, and great with people.

What your role actually is:

You’ll meet patients in hospitals and rehab facilities, explain our post-discharge program, and get them enrolled.

Patients are often pre-identified as strong candidates for a Medicare/Medicare Advantage-covered program, but not always pre-introduced — you must be comfortable starting conversations, building trust quickly, and guiding patients to enrollment.

This is a highly independent, field-based role — you’ll start your day at healthcare facilities meeting with physicians, nurse practitioners, and patients, and may also travel to patient homes for equipment installations as needed.

Primary focus = patient enrollment. Installation is secondary.

Success in this role = patients say YES and are set up quickly after discharge.

What you’ll do

Patient-facing (Facilities)

  • Receive a daily list of patients from physicians and PAs, with direction from the Soteria Project Manager
  • Meet with multiple patients per visit
  • Introduce the program (sometimes warm, sometimes “cold”)
  • Explain plan is covered (Medicare/Medicare Advantage) and program benefits
  • Answer questions and guide patients to enrollment
  • Obtain consent and complete enrollment in BlueStep EHR
  • Schedule installation prior to discharge
  • Coordinate with care team

In-Home Installation (As Needed)

  • Travel to patient homes across Tampa, Bradenton, and Sarasota
  • Install and test tele health system, vital sign devices, and personal emergency response system (PERS)
  • Teach patients/caregivers how to use equipment
  • Confirm everything is working before leaving
  • Complete documentation and notify team

What we’re looking for

Required

  • Reliable car + willingness to travel daily (mileage reimbursed)
  • Comfortable working independently in a field-based role
  • Strong communication skills — empathetic, confident, articulate, and appropriately assertive
  • Able to guide conversations and confidently ask for enrollment
  • Comfortable initiating conversations without a prior introduction
  • Proficient with email and messaging; keeps team informed
  • Comfortable with basic technology (training provided)
  • Must wear scrubs and maintain a clean, professional appearance
  • Dependable and patient-focused

Preferred

  • MA, CNA, or similar clinical background
  • Experience with seniors or post-acute patients
  • Bilingual (English/Spanish) a plus

  • Valid Driver's License

Pay: $25.00 - $30.00 per hour
