Patient Intake Coordinator - Virginia

The Patient Intake Coordinator plays a critical role in transitioning patients from referral to care by conducting initial outreach calls, gathering and verifying required information, explaining Personic Health’s services and care process, and scheduling the patient’s first visit. This individual must communicate clearly, compassionately, and confidently, directly influencing referral conversion, patient satisfaction, and timely access to essential wound care. This position reports to the Regional Vice President (RVP) and collaborates closely with back-office support staff to ensure a seamless patient onboarding experience.

Key Responsibilities:

  • Conduct initial phone outreach to newly referred patients in a timely manner
  • Collect, document, and verify demographic, insurance, and clinical information
  • Clearly explain Personic Health’s services, care model, and what patients can expect
  • Address patient questions and concerns with professionalism and empathy
  • Schedule initial visits and coordinate logistics with clinical and operational teams
  • Ensure accurate and complete data entry across required systems
  • Track and document referral status
  • Communicate proactively with the RVP and support staff regarding barriers to scheduling or conversion
  • Escalate clinical or operational concerns appropriately
  • Maintain confidentiality and compliance with HIPAA regulations

Qualifications:

  • Strong verbal communication skills and professional phone presence
  • High attention to detail and organizational skills
  • Ability to manage multiple systems and workflows throughout the day
  • Comfort working independently in a remote environment
  • Reliable high-speed internet and phone connectivity
  • Availability during standard work hours of 8:00 a.m. to 4:00 p.m. local time
  • Ability to learn and navigate software systems, including electronic medical record (EMR) tools
  • Familiarity and comfort with telephony systems (RingCentral, Nextiva, etc.)
  • Prior experience in healthcare intake coordination or patient access preferred
  • Experience in wound care, home health, or related healthcare services preferred
