Patient Liaison- Pharmacy

We are seeking a caring, professional, and highly organized Patient Liaison to serve as the bridge between our pharmacy, patients, caregivers, and healthcare providers. This role is focused on delivering an outstanding patient experience while helping patients navigate prescriptions, refills, prior authorizations, delivery coordination, and ongoing support.

The ideal candidate is compassionate, proactive, detail-oriented, and passionate about helping people.

Key Responsibilities

• Serve as a primary point of contact for patients and caregivers
• Assist patients with prescription onboarding and transfers
• Coordinate refill reminders and medication adherence follow-up
• Help patients understand copays, coverage, and next steps
• Communicate with prescriber offices regarding refill requests, authorizations, and clarifications
• Coordinate local delivery or shipment scheduling
• Resolve patient concerns professionally and promptly
• Maintain accurate patient communication notes and follow-up tasks
• Build long-term patient relationships through exceptional service
• Work closely with pharmacists and staff to ensure smooth workflow

Ideal Candidate

• Excellent communication and customer service skills
• Friendly, empathetic, and patient-focused attitude
• Strong phone etiquette and professionalism
• Organized with attention to detail
• Able to multitask in a fast-paced environment
• Experience in pharmacy, medical office, healthcare, or customer service preferred
• Pharmacy Technician license is a plus, but not required
• Bilingual is a plus
