Increasing enrollment, engagement, and retention in care of HIV/Hepatitis C positive patients accessing Emergency Department Services. Serve as a conduit for these patients to accessing medical care and medical specialty services as well as community resources. Additionally acts as primary community and financial resource for HIV/Hepatitis C patients.
EDUCATION/EXPERIENCE
ESSENTIAL FUNCTIONS
1. Screening & Assessment:
o Conducts risk assessments for HIV and Hepatitis C among emergency department patients using standardized screening tools.
o Identifies patients at risk based on clinical, behavioral, and social determinants.
2. Resource Referral:
o Connects patients who screen positive or are at risk to appropriate hospital-based or community resources for further testing, treatment, and support.
o Provides information on financial, emotional, and social support services.
3. Collaboration & Communication:
o Maintains close communication with emergency department staff, case managers, and medical social workers regarding patient needs and care plans.
o Participates in multidisciplinary case reviews to ensure coordinated care.
4. Barrier Reduction:
o Addresses barriers to screening and follow-up, including stigma, lack of knowledge, and logistical challenges.
o Develops strategies to re-engage patients who decline screening or miss follow-up appointments.
5. Documentation:
o Maintains accurate and timely documentation of patient interactions, screenings, and referrals per program policy.
6. Patient Engagement:
o Contacts patients to provide education, reschedule missed appointments, and encourage participation in recommended care pathways.
7. Education & Advocacy:
o Provides patient and staff facing education about HIV and Hepatitis C risk, prevention, and treatment options.
o Advocates for patient-centered care and health equity within the emergency department.
