Patient Navigator - Registered Nurse for Gender Affirmation Surgery Clinic

Job Overview
We are seeking a compassionate and highly skilled Registered Nurse to serve as a Patient Navigator in our Gender Affirmation Surgery Clinic. This vital role involves guiding patients through their gender affirmation journey with expert nursing care, ensuring seamless coordination of services, and providing empathetic support at every step. The Patient Navigator will leverage extensive clinical experience to facilitate patient assessments, coordinate multidisciplinary care, and promote positive health outcomes. Duties

  • Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations relevant to gender affirmation procedures.
  • Serve as the primary point of contact for patients, providing education about procedures, pre-operative preparations, and post-operative care instructions.
  • Coordinate with surgeons, anesthesiologists, mental health professionals, and other healthcare providers to develop personalized care plans aligned with each patient's needs.
  • Manage patient intake processes and facilitate scheduling of consultations, diagnostic evaluations, and follow-up appointments within the clinic.
  • Assist with discharge planning by ensuring patients understand medication regimens, wound care protocols, and necessary follow-up visits.
  • Maintain accurate medical records in compliance with HIPAA regulations and hospital policies while documenting all patient interactions thoroughly.
  • Monitor patients' progress during outpatient visits or inpatient stays, utilizing skills in infection control, trauma management, and acute care as needed.

Qualifications

  • Valid Registered Nurse (RN) license with current registration in the applicable state or region.
  • Must be able to speak Spanish
  • Strong background in medical-surgical nursing, outpatient clinics, or post-acute care environments; familiarity with behavioral health and geriatrics is a plus.
  • Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and discharge planning processes.
  • Demonstrated ability to perform patient assessments accurately while maintaining HIPAA compliance and confidentiality standards.
  • Experience working within hospital or clinic settings that involve infectious disease care or utilization management is advantageous.
  • Excellent communication skills combined with a compassionate approach to patient service; ability to coordinate multidisciplinary teams effectively. Pay: $78,000.00 - $130,000.00 per year
